Classroom Discipline Plan

Class Rules:

1.   Raise your hand for permission to speak or get out of your desk.

2.   Bring all necessary materials to class.

3.   Complete all assignment (class work and homework).

4.   Respect your teachers, classmates, and yourself.

Consequences:

1^st infraction:    Warning

2^nd infraction:   Documented student conference/ OTI (Opportunity to Improve) *enter in WebPams

3^rd infraction:    Documented OTI (Opportunity to Improve) and parent phone call or email *enter in WebPams

4^th infraction:   Teacher assigned consequence of a writing assignment which student must complete and return with parent’s/guardian’s signature  *enter in WebPams

5^th infraction:    Major referral and parent phone call *enter in WebPams

****Serious infractions can result in an automatic major referral****
